WAIVER OF LIABILITY FOR MUSIC TOGETHER IN SUSQUEHANNA VALLEY

As consideration for Music Together in Susquehanna Valley permitting me to take Music Together^® classes, I agree to the following: 

Liability Waiver: I agree not to hold Music Together in Susquehanna Valley, Gloria Mann, and its representatives responsible for injuries or accidents in connection with Music Together in Susquehanna Valley practices or performances on or off-site. If any injury appears serious enough for hospitalization or medical attention, such expense will be borne by the parents or guardian. In case of injury or illness, I grant permission to any available physician to perform emergency treatment for my child(ren)/ward(s) as deemed necessary. I agree that Music Together in Susquehanna Valley is not responsible for personal items (including instruments and cell phones) broken, brought, or left at any Music Together in Susquehanna Valley teaching facility or event (including but not limited to classes, playdates, etc.). I understand and voluntarily assume all risks inherent in the nature of this activity and I waive and release all claims, costs, liabilities, expenses and judgements against Music Together in Susquehanna Valley and its representatives arising out of my participation or my child’s participation in Music Together in Susquehanna Valley activities. By enrolling in classes, I agree that I have read this document and I hereby waive and release Music Together in Susquehanna Valley and any contracted teachers or independent contractors, from any liability for any injuries and illness incurred while my child(ren)/ward(s) under my care or I are at Music Together in Susquehanna events. You MUST be 18 years or older to enroll in classes indicating that you have read this waiver. You MUST be the Parent or Legal Guardian to sign for a minor (under age 18). 

By Registering for class you agree to these terms and policies and our liability waiver. 

I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS.

BY ENROLLING IN CLASSES AT MUSIC TOGETHER IN SUSQUEHANNA VALLEY, I UNDERSTAND THAT I AM AGREEING TO THE ABOVE AND HAVE GIVEN UP CERTAIN LEGAL RIGHTS. THIS IS A BINDING LEGAL DOCUMENT.
